Does the 'full heal' capability of this special energy card only work
the first time it is played or does it work like a power and keeps the
pokemon healed the whole time it is in play?

I appreciate any direction you can give.

This Energy, along with Potion Energy both provide a one-time effect that happens when the card first comes into play.  Once the energy is on the Pokemon, the special effect of the card is gone and it becomes a single colorless energy.
